  • Marketing, outreach, relationship management with professionals and use of risk stratification
  • Screening and risk assessment of individuals
  • Delivery of programmes in relation to Weight Management, Physical Activity and Falls Prevention, Smoking(specialist and community), Mental Wellbeing (brief intervention) as well as Innovation Projects.
  • Case management of participants and tracking of outcomes via an appropriate IT system.
