Specialist care for persons with long term clinical conditions;
Specialist care for older people;
Multidisciplinary/multi-agency assessment and care management planning;
Return to home and follow-up care arrangements;
Active, goal-based rehabilitation, enabling people as far as possible to return to their previous level of function and independence;
Intermediate/integrated care;
Post-operative rehabilitation and recovery;
Medical assessment and treatment;
Continued specialist therapy;
Investigations to aid diagnosis; and
Palliative and terminal Care.
