West Berkshire Council Public Health and Wellbeing Team are hosting a market engagement event for the delivery of a Community Wellness Outreach Service with a primary aim of Cardiovascular Disease (CVD) prevention. The service will deliver the following in the community setting:(1) An enhanced NHS Health Check offer supplemental to the current Universal NHS Health Check offer that is currently delivered by Primary Care. This will be delivered to population groups that are aged 40-74 years, who are underserved by the existing NHS Health Check offer, and disproportionately impacted by the burden of cardiovascular disease.(2) A holistic "health check" offer for individuals aged above 18, who are ineligible for the NHS Health Check, but may benefit from brief intervention to support with risk factor modification through behaviour change.(3) An integrated specialist social prescribing offer.(4) Collaborative working with Primary Care partners to develop and implement a health data management solution.A previous market engagement event was held in 2022 which informed the development of a specification for a NHS Health Check Targeted Outreach service. Since then the scope of the service to be contracted has broadened to include a wider population and remit, however the focus remains on delivering a targeted service for cardiovascular disease prevention, with the aim of reducing health inequalities.
