Improving Access, Reducing Health Inequalities, and Facilitating Choice: • The service guarantees equitable access to timely medical care for all D2A patients. • Flexible consultation models support patient choice and accessibility. • By ensuring regular GP input and proactive care planning, the service helps reduce unnecessary hospital readmissions caused by unmanaged chronic conditions, medication issues, or deterioration in frailty patients. • The service aligns with NHS priorities around integrated, patient-centred care by ensuring that patients discharged into community settings maintain continuity of medical oversight. • Personalised care planning ensures that patients and families are involved in decision-making about long-term care needs. 5. Social Value: • Providers actively contribute to local workforce sustainability, supporting long-term community healthcare resilience. • Digital transformation initiatives reduce administrative burden and environmental impact. • GP-led interventions strengthen care home capacity and patient well-being. • Early intervention models improve patient outcomes, reducing reliance on emergency hospital care. Quality and Innovation: • Providers are expected to sustain high-quality, responsive care, with continued compliance against KPI and CQC standards. • Further investments in Primary Care digital monitoring tools and electronic patient records will enhance service accessibility and efficiency. • Ongoing primary care clinical training and governance improvements will ensure continuous improvement in care quality. 2. Value: • The remuneration model will continue to provide stability while reducing pressures on acute services. 3. Integration, Collaboration, and Service Sustainability: • Providers will deepen collaboration with system partners, embedding enhanced multi-agency working models. • Service integration will ensure resilience, adaptability, and alignment with evolving NHS priorities. 4. Improving Access, Reducing Health Inequalities, and Facilitating Choice: • Expanded digital consultation models will enhance patient choice and accessibility. 5. Social Value: • Ongoing investment in workforce development will further strengthen local healthcare provision. • Sustainability initiatives will continue to reduce the environmental impact of service delivery.
