This is a Direct Award C the NHS Provider Selection Regime to the following 9 Primary Care Trusts and GPs that form them

AT Medics PCN: Streatham High Practice and Edith Cavell Practice
Brixton and Clapham Park PCN: Clapham Park Group Practice, Hetherington Group Practice and Hetherington at the Pavilion.
Clapham PCN: Clapham Family Practice, Grafton Square Surgery, Dr Curran & Partners, and Sandmere Practice.
Croxted PCN: Paxton Green Group Practice, The Old Diary Health Centre, and Brockwell Park Surgery.
Fiveways PCN: Vassall Medical Centre, Akerman Medical Practice, Minet Green Health Practice, Herne Hill Road Medical Practice and The Corner Surgery.
HBD PCN: North Wood Group Practice, Herne Hill Group Practice, Brixton Hill Group Practice, the Deerbrook Surgery, Prentis Medical Centre, and Knights Hill Surgery.
North Lambeth PCN: Waterloo Health Centre, The South Lambeth Road Practice, The Vauxhall Surgery, Mawbey Group Practice, Lambeth Walk Group Practice and Hurley and Riverside Practices.
