What is being bought

Published requirement description

Cardiff and Vale University Health Board, intends to award a contract to Medivate for the one off purchase of Geister Peripheral and Major Vascular Access Instruments. The University Hospital of Wales (UHW) operates as a Major Trauma Centre and provides regional vascular and trauma services, receiving referrals from across South and West Wales. Theatre capacity is being expanded, including the development of additional major trauma and hybrid theatres to support high‑complexity trauma and vascular surgery.Peripheral and major vascular instrument sets are required for use across multiple theatre settings, including elective vascular theatres, CEPOD emergency theatres, Major Trauma Centre theatres, and hybrid/endovascular suites. These instrument sets support the delivery of abdominal aortic repairs, carotid endarterectomies, lower limb revascularisation procedures, trauma‑related vascular repairs, and conversions from endovascular to open surgery. The existing instrument inventory currently comprises three major vascular sets and four peripheral sets, which are fully utilised for elective, endovascular, and reserved damage‑control activity. This utilisation limits availability for urgent and out‑of‑hours procedures. The procurement of additional instrument trays is required to maintain operational capacity, ensure cover for emergency, weekend, and trauma activity, and support continuity of elective services across planned and unplanned care pathways.

Published justification

To maintain standardisation of both major and peripheral vascular access instrument sets across theatre environments, the use of Geister instruments is required. These instruments are necessary to ensure consistency of configuration and compatibility with the existing instrument inventory in use within the Health Board. Medivate is the sole authorised distributor of Geister vascular instruments within the UK, and no alternative suppliers are available to provide equivalent items.

Review and risk notes
Risks

Instrument capacity and service resilience risk

Current instrument capacity is sufficient to support routine elective and endovascular activity but provides limited resilience for emergency, CEPOD, and Major Trauma Centre activity, particularly out of hours or following weekend use. A lack of available trays at peak times presents a risk to timely service delivery.

Clinical and equipment reliability risk

Existing capacity constraints increase reliance on frequent instrument cycling, creating pressure on available sets and contributing to an identified clinical risk associated with limited equipment availability and resilience. This has been assessed as a service-critical risk within existing risk management processes.

Decontamination and turnaround risk

While decontamination capacity is approved and available, the limited number of trays places pressure on turnaround times within the Hospital Sterile Services Department. This may result in workflow bottlenecks and reduced flexibility during periods of increased demand.

Training and operational transition risk

The introduction of additional trays requires familiarisation and training for operating theatre and decontamination staff. Although training will be delivered internally, there is a short-term risk of operational inefficiency during the transition period.

Asset disposal and compliance risk

The replacement of existing instruments requires compliant disposal in line with Clinical Engineering and asset management processes, although no data security risks are associated with the equipment.

Mitigation

The procurement of additional major and peripheral vascular access trays will increase instrument resilience, support 24/7 service availability, reduce reliance on rapid cycling, and improve decontamination workflow. Standardised tray configuration, internal training delivery, and managed disposal arrangements will mitigate operational, clinical, and compliance risks.
