It is an element of the Acute Stroke Pathway in Stockton
on-Tees, which also includes the Community Stroke Team (CST) commissioned by the North East and North Cumbria Integrated Care Board (ICB) as part of the Community Integrated Assessment Team (CIAT). - Conduct regular reviews of each stroke survivor's needs.
- Guide people using the service through lifestyle changes based on
identified risk factors and provide stroke survivors and their families and/or
informal carers with practical support and advice.

activities that are relevant to them as individuals, and aid the reduction of
social isolation.
- Make appropriate onward referrals where additional needs are identified
and with the consent of the person using the service.
