Who may buy through it
Southend City Council (SCC) are looking to establish a Care at Home Framework in which any statutory funded organisation can utilise.
Published description of the framework
Costs:
Costs are established using fixed rates of which all providers agreed to during the procurement and contract signing process.
This Framework Rates will be agreed every March, following the
Annual Rate Review.
Allocation of Work:
Lead providers are responsible for delivering high-quality care
services within their designated geographic locality. They focus on providing homecare, reablement, and enhanced discharge services directly to individuals .
Locality providers operate within smaller, defined geographic
areas. They provide care services to individuals within these areas, ensuring that local needs are met.
Process:
- People coming through the hospital discharge pathway will go via SEDs/Reablement to Mainstream with the lead provider for that area
- Lead provider is expected to provide Mainstream services for people in this pathway up to the agreed block hours. If the block hours are met and the lead provider unable to accommodate, the client provision will be sourced separately by brokerage.
- People who come from the community via brokerage will be offered firstly to the lead provider in an area. If the block hours are met and the lead provider unable to accommodate, the client provision will be sourced separately by brokerage.
- Brokers will offer the clients package of care to those providers who expressed a preference for a specific locality geography and were awarded a position as locality provider based on their preference (lots 4 - 7)
- Should this not achieve a placement the brokerage will then open to the balance of locality providers and other lead providers.
